School Nurse Info

The school nurse is available from 8:30 – 2:30 during the week to help students address their health needs. She disperses prescribed medicine to students, hold emergency contact information, and treats injuries and illnesses as they develop during the school day. Students must have a pass signed by their teacher to visit the nurse; her office is on the 2nd floor of the building.

Immunizations & Physicals

*Please be reminded that ALL students need to have a recent Physical and a copy of Immunizations on file in school. Immunizations - Boston Public Schools  

*Please send in a copy of your students current physical and immunizations on the first day of school!

Student Athletes:

All athletes require a physical examination completed within the last 13 months that does not expire before the end of the season, in order for the nurse to clear them to play.

Medical Conditions & Medications

If your child has any Medical Conditions *Please let me know as soon as possible. 

  • If your child has medications that need to be given at school please send in a signed order from your physician along with the medication in the original bottle. The Medication authorization Form can be completed by your physician.                     

  • If your child has asthma please have your physician fill out the Asthma Action Plan and send the plan into school as well as the medication. See an example of an Asthma Action Plan.

  • If your child has a seizure disorder please send in the seizure plan from your physician as well as the medication. See an example of a Seizure Action Plan.

  • If your child has a Life Threatening Allergy that requires an EPI-Pen. Please send in physician order and medication as soon as possible

  • If your child needs over the counter medication at school please fill out this form and return to the school nurse.

Please have all your forms completed and medications ready for the first day of school.


Field Trips

if your student plans on attending any field trip this year and is prescribed any medication (ie. Asthma pump or epi-pen for allergies) your child will not be able to participate unless the medication is in school along with an action plan from the prescribing doctor. Students going on overnight trips need to have a current physical exam and immunizations need to be up to date.

Health Center

New Mission is lucky to have a Health Center on site. If you would like your child to be seen by the Nurse Practitioner during the school year  please fill out this consent form and return to the Health Center.

Boston Public Health Commission

BOSTON PUBLIC HEALTH COMMISSION: BOSTON COMMUNITY LEADERSHIP ACADEMY AND NEW MISSION HIGH SCHOOL

The Boston Public Health Commission School Based Health Centers (SBHC) provide health care services for youth in 8 Boston Public High Schools. Services address medical and social health concerns that frequently interfere with academic success. Teams include a Nurse Practitioner, Mental Health Clinician, Health Educator and Health Coordinator/Administrative Assistant who work together to assure enrolled students have access to the information and services they need to remain healthy. Services offered include:

  • Medical: Nurse practitioners provide preventive care, routine annual sports/employment physical exams, diagnosis and treatment of episodic illness (like strep throat), immunizations, chronic illness care (including asthma) and sexual/reproductive health education and management (including STI testing/treatment).

  • Mental Health: Licensed Mental Health Counselors or Licensed Clinical Social Workers provide the following: Psychosocial assessment and case management for individuals or groups; intervention in crisis situations; provision of information, referral and resources for individual, family, and school personnel within the school and community. SBHC clinicians also work in partnership with schools in response to traumatic events.

  • Sexual Health/Reproductive Health: Sexual/Reproductive Health educators provide one-on-one counseling and classroom education to high school students, including providing information on healthy bodies, healthy relationships, and prevention of STIs and unplanned pregnancies.

A feel good moment from the world of PE

All of our students will participated in the FitnessGram Pacer multistage test that challenged their aerobic capacity, cardiovascular endurance, flexibility, muscular strength and muscular endurance. Our High School students were cheering on and supporting our Middle School students.
